The Office of Vital Statistics ONLY issues birth certificates to persons who establish themselves as the subject of the birth record, the subject’s parent, legal guardian, legal representative, spouse, child, grandchild, or sibling if of legal age.
To request a birth certificate, you must provide the following:
- The full name as it appears on the birth record, the date of birth, the mother’s full maiden name, and the father’s full name
- Proof of identity in the form of photo identification such as a photo driver’s license or passport. If you do not have a photo driver’s license or passport, you must provide two alternate forms of identification
OR
- A current, valid driver’s license without photo and one alternate form of ID.
OR
- Two alternate forms of ID.

- Vehicle Insurance Card • Voter Registration
- US/Foreign Passport
- Immigrant Visa
- Permanent Resident Card (Green card)
- Federal, State and County ID
- School ID
- Utility bill (within the previous 90 days)

The fee per certificate is $25.00. No personal checks or cash accepted. The fee for additional copies of the SAME RECORD on the SAME DAY is $5.00.

The Division of Health offers High Impact HIV Prevention.
This is an evidence-based public health approach to HIV prevention that applies cost-effective and adaptable interventions in serving the public. Rapid HIV (Human Immunodeficiency Virus) testing and counseling are done on demand at the Paterson Division of Health clinic Monday through Friday during the hours of 9:00 a.m. to 3:30 p.m. No appointment is necessary. Additionally, we operate a mobile medical unit as part of an outreach effort during normal work hours as well as non-traditional hours, so that those unaware of their HIV status can be tested and learn what their HIV status is.
Clients receive:
- Counseling and testing with test results in 20 minutes
- Prevention Counseling
- Early linkage to primary medical care for those who test positive for HIV
- Referral to dental care and other services for those who test positive for HIV
- Referral to pre-exposure prophylaxis (PrEP) program for those with very high risk of HIV infection
- HIV-related outreach activities and testing incentives

To request a marriage certificate, you must provide the following:
- The full name of Spouse A, the full name of Spouse B, date of marriage, and place of marriage (Street address, city and state)
- Proof of identity in the form of photo identification such as a photo driver’s license or passport. If you do not have a photo driver’s license or passport, you must provide two alternate forms of identification such as:

Spouse A or Spouse B must make an appointment to apply for a marriage/civil union license/domestic partnership.
At least one applicant must provide proof of residence in Paterson. (For example: a telephone or cable bill)
Applicants must have one witness over the age of 18 present, who knows how to fully speak and understand English language.
Appointments must be made in person and scheduled 3 months prior to wedding event.
The fee per license is $28.00. Money orders or debit card payments ONLY. (Cash or personal checks are not accepted)
For individuals who are not infected with HIV but are at a very high risk of infection from sexual activities or injection drug use, pre-exposure prophylaxis, or PrEP, is an evidence-based intervention to help prevent HIV infection.
We are a one-stop center for PrEP. All counseling, medical consultation and prescription services are offered in our clinic Monday through Friday during the hours of 9:00 a.m. to 3:30 p.m. An appointment is not required.

The Paterson Division of Health operates a Sexually Transmitted Disease clinic on Mondays, Tuesdays, Thursdays and Fridays from 8:45 a.m. to 1:00 p.m.
Clients ages 13 and older are screened for free, for:

Paterson Division of Health in partnership with St. Joseph’s Hospital and Medical Center provides free care and treatment as needed. Comprehensive STD education with risk reduction counseling, and partner notification, are also provided as part of our program. We work closely with the New Jersey Department of Health Sexually Transmitted Disease Program. No appointment is needed and clients are seen on demand.
